Frequently Asked Questions
How do I request immunization (shot) records from the Health Care Agency?

Get the Card in Person: Go to the Immunization Clinic and request an additional copy. You can fill out an authorization form at the clinic and the clinic will give another copy to you at that time.
Get the Card by Mail or Fax: Download and fill out the authorization form "Authorization to Use and Disclose Protected Health Information". Once the form is completed and signed, please return the form to our office by mail or fax. There is no fee for this request. It usually takes between 7-10 days for you to receive the information by mail.
